On August 1, 2024, an employee was working on a press and dumping scrap material from the center piece to a scrap barrel when their finger became caught in the hole of the center piece and was crushed between the center and the edge of the roller on the conveyor. The employee sustained an amputation of three-quarters of their finger.

An employee was using a press, a spacer, and a lift. The press and the spacer were being used together but were not permanently affixed to each other. At the end of the order, the employee clamped the spacer and the press together with a C-clamp and tried to remove them from the operation by flipping the press over. His fingers were pinched in the gap between the press and the spacer next to the C-clamp.

HB Fleming, Inc.

A crew was installing drilled micropiles alongside an outdoor covered deck foundation. The injured employee was working the front of the drill when a loose section of casing dropped onto the tip of a rig wrench and pulled the wrench down to an embedded casing. The employee's left little finger was caught between the rig wrench and the embedded casing and was amputated above the top knuckle.

Howard Industries Inc.

An employee was checking a pole-type transformer tank for leaks. A hydraulic clamping device was lowered onto his right thumb and crushed it against the sharp edge of the bottom of the tank. He suffered a partial transphalangeal amputation to the thumb.

3S Services, LLC

An employee had been cleaning the work area. While lifting a saw, the employee's index finger was lacerated by the saw blade. A tendon was cut and required surgery.

An employee was installing a hydraulic extension cylinder in a tow truck when their leg was caught between the cylinder and the truck frame. The employee was hospitalized with a broken tibia, which required surgery.

An employee was standing on a step ladder and operating a hydraulic jack. The jack began shifting, causing the ladder to tip to the left. The employee caught himself as he was stepping down from the third rung of the ladder. When he stepped to the floor, his left foot rolled and twisted under his body weight, resulting in fractures to his tibia and fibula above the ankle.

An employee was removing grating to change metal chip hoppers located in a pit. The employee lost his balance and fell approximately 12 feet from the grate, landing in the metal chip hopper. The employee sustained a sprained right foot, a left foot fracture, and a fractured L5 vertebra.

An employee was using a hammer to hit a steel bearing and dislodge it from a shaft. A small piece of the bearing broke off and embedded in his stomach.

An employee was exposed to carbon monoxide from a forklift and began to feel lightheaded. The employee was later was found unresponsive in the booth and was hospitalized.

An employee fell while moving a water canister, resulting in fractures to four fingers on her right hand.

An employee was changing a die in a press when the die slipped and crushed the employee's left index finger. The employee sustained an open facture of the tuft of the left distal phalanx and a partial amputation.

An employee was driving a boom lift (in the lowered position) in an exterior dock area. The lift s left wheels rolled onto base plates that covered a 3-foot-deep sump pit. The base plates failed, and one side of the lift dropped. The employee's left leg was caught under the lift basket, and he suffered a fracture to the lower leg including the ankle.
