At the end of the order, the employee clamped the spacer and the press together with a C-clamp and tried to remove them from the operation by flipping the press over. His fingers were pinched in the gap between the press and the spacer next to the C-clamp.

The injured employee was working the front of the drill when a loose section of casing dropped onto the tip of a rig wrench and pulled the wrench down to an embedded casing.
