An employee was lifting the cab off a skid steer. As the cab was being lifted and tilted back, the cab door swung down and pinched the employee's right ring finger resulting in a partial amputation.

An employee was fixing a link on a track. The track pads shifted and pinched the employee's left pinky finger. The finger was later amputated.

An employee was working on extracting an angle cylinder pin from an 850K bulldozer when a piece of metal separated from the pin and cut the employee's wrist.

The crane then sent the beam southward, directly toward the corner of the building. The beam caught the tips of the employee's left index, middle, and ring fingers against the building. The last joints of the middle and ring fingers were amputated, and the last joint of the index finger was partially crushed.

An employee was placing a board on top of a bundle of steel tubing. A crane was moving a bundle of steel that struck the employee and caught them against another bundle of steel tubing. The employee sustained a crushing injury and hematoma to the right upper leg.

Employees were using a crane to lower large tree stems and branches while removing limbs and branches from the load. This caused the load to shift and strike an employee in the back, resulting in a hematoma on the lower left part of their back and a fractured ankle.

An employee was operating an ironworker (steel punching/shearing) machine. A piece of the tooling broke off and struck the employee's neck; a fragment was lodged in his neck. The employee was hospitalized and required surgery.

An employee was climbing a ladder to inspect a vent in a restaurant dining area. The ladder slid out on the floor and the employee fell approximately 12 feet onto the ladder and the floor. He was hospitalized with a dislocated shoulder and a back injury.

An employee was operating a lift truck when it struck a stationary lift truck. His left thumb was pinched between the two lift trucks, resulting in a partial amputation.

An employee was disassembling a pressurized unit using a driver drill. When the unit was disassembled, it caused a kickback from the tube head, causing the drill to strike the employee's left forearm. They sustained a diaphyseal fracture to their left radius and ulna, which required surgery.

After repairing a piece of industrial equipment, an employee had brake-cleaning liquid on his gloves. A lighter ignited the liquid, and the employee's hands were severely burned.

After cutting a slab of beef short ribs, an employee turned to grab the pieces he had cut and his right hand contacted the saw blade. The employee sustained an amputation to his right index finger.

An employee was unloading a carpet pad from a truck when they fell from the truck dock to the concrete below, resulting in five fractured ribs and an injury to their left lung.
