An employee in the rail yard was struck by the corner of a slow-moving railcar and lost their balance. The employee grabbed onto the side of the railcar to regain their balance and sustained an avulsion injury to the left thigh and a broken right ankle.

Borealis Compounds, Inc.

Two employees were moving a railcar (plastic pellet hopper car) using a mobile railcar mover. One employee was the engineer in control of the railcar mover and the injured employee was a switchman. The railcar mover went forward and the hopper railcar struck the injured employee, resulting in a severe crushing injury to the employee's right arm.

DUNN ROADBUILDERS

An employee was working the switchboard for a train to move through the plant. He gave the all clear signal, and the train began reversing on the tracks. The employee went to get onto the train as it was moving toward him when he missed the railing and fell underneath the train. The last railcar ran over both of his legs, amputating the legs above the knees.

Archer Daniels Midland Company

On April 27, 2025, employees were performing a railcar switch. When the train began to shift back, the injured employee's right arm got crushed between the knuckles of two railcars. The employee was hospitalized with fractures to their forearm.

Tri-Lakes Services Inc

An employee was working to couple two railcars together, holding the lever with one hand and the coupler with the other hand. The locomotive then backed up and his left arm got caught in the coupler, resulting in a broken arm and a broken little finger.

Azcon Metals, Inc.

An employee was uncoupling railcars while the cars were in motion and a car ran over her left lower leg, resulting in crush injuries and amputation below the knee. The employee was hospitalized.

An employee was cutting a piece of metal rod with a metal cutting machine. His right middle finger became caught between the rod and the machine's table. The employee's fingertip was partially amputated.

CSBS Manufacturing LLC

Two maintenance employees were dismantling a machine part (steel cylindrical rollers) on the ground. After unbolting a portion, part of the assembly fell approximately 6 inches onto one employee's left little finger. The employee was hospitalized and about 3/4" of their little finger above the top joint was surgically amputated.

An employee was moving and handling cardboard boxes when he sustained a laceration to his left ring finger from a box. The laceration became infected and required hospitalization and surgery.

An employee was positioning a piece of metal trim for a test bend in a single-fold bend machine. Their right middle fingertip was pinched by a clamp on the machine, resulting in a partial amputation.

An employee was operating a double folding machine and positioning a piece of metal trim in the folding clamps for a test bend. The machine activated and the crank (lower control arm that bends the trim) came up and cut through the employee s glove, amputating their right middle fingertip.

A driver was exiting his vehicle outside the plant gate when his foot slipped on the top step of the truck. He fell to the ground, landed on his left hip and elbow. The employee was hospitalized with a fractured hip/femur.

CHRISTUS St. Frances Cabrini Hospital

A security employee was responding to an urgent call from staff regarding a violent patient. The employee tripped and fell on the floor outside of the stairwell. The employee sustained a closed head injury, contusion of the cerebrum without loss of consciousness, and a closed fracture of the distal end of the right radius.

Jean Simpson Personnel Services, Inc.

A temporary employee was testing an electrical starter motor. He was placing tape on the starter while the breaker was not engaged, but the starter sent an arc flash that burned his hands and stomach.

An employee was walking on a sidewalk and stubbed his toe on an elevated portion of concrete, causing him to trip and fall. The employee's right knee was dislocated.
