An employee was working the switchboard for a train to move through the plant. He gave the all clear signal, and the train began reversing on the tracks. The employee went to get onto the train as it was moving toward him when he missed the railing and fell underneath the train. The last railcar ran over both of his legs, amputating the legs above the knees.

The first employee was operating an asphalt paver while a second employee was walking to the left of the paver, examining the paving, when a publicly owned vehicle veered into the designated paving area and struck the two employees. A concrete batch plant operator was assisting with clearing spoil piles using a skid steer. The skid steer backed into a stationary screen plant. The employee's left little finger was crushed between the controls of the skid steer and the screen plant, resulting in a fracture and laceration. The employee's finger was surgically amputated.

This caused the employee's hook to slide down and pinch his right index finger between the chain and the shackle.
