Final narrative
An employee was operating a horizontal band saw. When he reached for the piece that he had just cut, his left hand contacted the switch that controls the vise clamp on the saw. The vise clamp engaged and closed on his left little finger. The employee sustained a finger amputation.

An employee was operating a metal roller machine to roll a small piece of metal. The employee's right index finger became caught between the metal and the machine, and the fingertip was amputated.
