An employee was performing maintenance on a turret. They removed a panel to view the turret's bearings, and their left hand was pulled into the rotating turret, resulting in a fracture.

An employee was monitoring and operating a converting machine. As the employee was removing a roll of plastic trash bags that was stuck on a shaft, the machine's push-off device/plate lacerated the top of the employee's right hand.

An employee was filtering a fryer with a fryer filter machine. After going around the corner and then returning to the fryers, the employee stepped into the filter machine. The hot oil burned the employee's right ankle, and the employee was hospitalized.

An employee was using a water hose to clean debris out of the outfeed of a log conditioning vat. Water began entering the vat from the adjoining vat through a void in the separation wall at the infeed of the vats. As the employee was exiting the vat he had been working in using the access opening at the outfeed, hot water exiting the access opening entered the top of his protective hip wader. It pooled at the bottom of the wader and burned his left foot and ankle.

An employee was operating a drill and tap machine. He was placing a part with his right hand and removing a part coming in on the left side. He saw a part that was crooked and went to straighten it. The machine clamped down on his left index finger, degloving the top part of the finger and resulting in a partial amputation.

An employee was performing maintenance on a roll cutting machine in a warehouse. The employee was removing excess grease from the bearings when the machine's chain caught their right index finger. The fingertip was amputated by the chain and sprocket.

During fiberglass work, an employee mixed a methyl ethyl ketone peroxide (MEKP) catalyst with a cobalt chemical that they thought was a dye. A chemical reaction occurred, resulting in an explosion. The employee sustained second-degree burns on his back and a blister on his right arm.
