An employee was changing out a die on an ironworker machine. The machine engaged, and the employee's index finger was caught between the plate stripper and the material rest. The fingertip was amputated.

After repairing a piece of industrial equipment, an employee had brake-cleaning liquid on his gloves. A lighter ignited the liquid, and the employee's hands were severely burned.

An employee was welding lift arms when their shirt caught fire due to welding sparks or heat, resulting in thermal burns to their upper chest.

An employee was cleaning a seam with weather membrane cleaner. The cleaner soaked into the employee's gloves, and they caught fire when a cigarette was lit. The employee suffered first- and second-degree burns to both hands and was hospitalized.

The injured employee had been working with xylene in a paint booth and exited the paint booth for a smoke break. The employee dropped his lighter, causing his clothing to catch fire, possibly from xylene vapors. The employee sustained burns from their chest down to their thighs and was hospitalized.

An employee was charging the furnace when a spark from the furnace ignited and struck the employee's undershirt. The employee sustained burns to their lower back, hands, and front abdomen.

An employee was acting as a spotter for a forklift operator. While its forks were being raised, the forklift came into contact with a power line. The employee was touching the forklift's metal frame at the time and was shocked on the left palm. The employee suffered burns to both the left palm and the sole of the left foot.

An employee was leaning down to install conduit. When he stood up, a 2-inch all-thread rod caught and lacerated his left eyelid.

An employee was working as a fire watch. The employee was exposed to anhydrous ammonia and was not able to see while climbing down a ladder due to eye irritation. The employee fell and was hospitalized for a broken ankle.

An employee was standing on the ground awaiting the lowering of a pipe. After the pipe was lowered to the ground by a skid steer, it rolled and struck the employee's left leg. The employee was hospitalized with a fractured leg.

An employee was tightening a gas pipe with a wrench when the wrench slipped from his hand. The employee's arm then hit a metal cross-brace, resulting in a lacerated arm.

A flagger was directing traffic flow in a highway work zone. They were standing approximately 1.5 feet behind the shoulder's demarcating line, next to and slightly behind a traffic drum. As they were directing traffic into and out of the driveway of a parking lot, a car attempted to get around a truck and struck the traffic drum, which subsequently struck the employee, who was thrown 15 feet and landed on top of concrete aggregate. The employee suffered pelvic and rib fractures, T10 and L3 vertebra fractures, and internal bleeding.

An employee was troubleshooting a power washer in the field. Because there might have been water in its fuel, he brought it back to the shop and drained about a gallon of fuel from the tank into a plastic container. Some of the fuel spilled onto the floor and ignited. The employee was stomping out the fire when he lost his balance and tripped into a stool, which caused the plastic container to spill more fuel onto the fire. The employee's pants and shirt caught on fire, and he fell, abrading his knee while trying to get through the flames. As well as the knee abrasion, he suffered burns to the left leg and left lower quadrant of the torso. He was hospitalized.

An employee was backing up a tram (towing powered industrial equipment) to connect it to a trash bin. The employee's left forearm/wrist was caught and crushed between the tram and the bin. The employee was hospitalized.

An employee went to lift a carton and tripped on a different carton that was on the floor. She fell on the floor in the backroom and sustained a fractured right hip, and abrasions to her arm and knee. The employee was hospitalized and required surgery.

An employee was conducting a delivery of beverage products and began having cramps due to dehydration. The employee was hospitalized.
