An employee was standing next to a transfer table which lifts tubing into the area to be processed and cut by the horizontal band saw. A 48-foot-long piece of 6-by-6 tube that was on the transfer table struck his right arm, resulting in an injury.

An overhead crane was being used to stack 4-by-4-by-20-foot steel beams on an existing pile of beams. An employee was walking through the area when a stack of about 20 beams from the pile collapsed and struck the employee's legs, breaking both of them.

An employee was hooking up a set of 28-foot trailers, attaching a converter dolly to the lead trailer's pintle hook. He backed up the lead trailer and parked it a foot from the converter dolly, then lifted the converter dolly and rolled it to the lead trailer to set it onto the hook. The converter dolly kicked up, and the employee's left hand was caught between it and the trailer. He suffered a severe laceration to the palm between the ring finger and middle finger.

An employee in the warehouse was working to pull a reel from under a rack when his finger was crushed between the reel and the rack. The employee sustained a fingertip amputation.

An employee was pushing automotive suspension material using a material rack. The material started to shift and caught the employee's right little finger, crushing it. The employee suffered an amputation to the finger. The finger was re-attached.

An employee was entering a receiving area to start his shift when he was struck by a utility cart being pushed by another employee. The injured employee fell to the floor and suffered a fractured hip.

A temporary employee was moving an I-beam that was on a steel table to the edge of the table. Their right index finger was pinched between the I-beam and the table, resulting in a broken finger.

A load of steel angles was being lowered when the lifting chain hit the load, causing it to slide toward an employee. An angle slid and pinched the employee, catching both of their legs between the beams and an angle. The employee suffered two fractured legs.

An employee was on the deck of a trailer helping a customer unload tubes. The employee was working to place a cargo strap on the remaining tubes using a winch bar when it slipped off the tubing, causing him to lose his balance and fall headfirst onto the asphalt lot below. The employee suffered a contusion on his forehead, resulting in hospitalization.

An employee and a customer were unloading two rectangular tubes. The tubes were wrapped together. The plastic wrap was cut so that one tube could be picked up. As the employee was lifting the top tube, the other tube (still connected to the top tube) came down onto his hand and broke two of his fingers. He was hospitalized.

An employee was assisting as a forklift got pipe from a pipe rack. The employee was on one of the forklift's forks as it was raised to the top of the rack, a height of about 15 feet. He fell to the ground and suffered a dislocated and fractured right elbow.

On September 23, 2025, at approximately 7:43 p.m., an employee was using a knife to cut poly banding from a metal coil. The knife slipped, causing a laceration to the inside of their left arm at the elbow joint. The employee was hospitalized and required surgery to repair a damaged vein.

After cutting a slab of beef short ribs, an employee turned to grab the pieces he had cut and his right hand contacted the saw blade. The employee sustained an amputation to his right index finger.

An employee was unloading a carpet pad from a truck when they fell from the truck dock to the concrete below, resulting in five fractured ribs and an injury to their left lung.

An employee was inspecting flares processes. The employee received burns to the front side of the body, face and arm from the flares.

An employee was removing plastic material from a production line when the machine cycled and amputated his left index, ring, and little fingers.

An employee was walking up a walkway to enter a building through the back entrance when she tripped over the lip of the cement ramp. The employee fell and sustained a fractured right hip.
