An employee was spotting for another employee who was loading a tile installation plow onto a trailer. The spotter was struck by the back filler brace when it broke off and fell from the plow. The spotter suffered a skull fracture and upper back injury.

A temporary employee was carrying a nail gun through the warehouse when it hit his leg, activated, and fired a nail into the leg.
