29 CFR 1910.303(b)(1): Electrical equipment was not free from recognized hazards that were likely to cause death or serious physical harm to employees:    On or about October 24, 2012, a 100-foot black extension cord was not inspected prior to being put into service. The cord had eight different cuts on the outer insulation exposing the inner conductor insulation to possible damage. The employees were exposed to electrical shock.

29 CFR 1910.305(g)(2)(iii): Flexible cords and cables were not connected to devices and fittings so that strain relief was provided that would prevent pull from being directly transmitted to joints or terminal screws:    On or about October 24, 2012, the eletrical cable was not connected to the fitting so that strain relief would prevent pull from being directly transmitted to joints or terminal screws.
