29 CFR 1910.304(g)(5): Grounding path. The path to ground from circuits, equipment, and enclosures shall be permanent, continuous, and effective:  On or about August 6, 2014, and at times prior thereto, the mud loggers trailer was not grounded properly. The ground wire was not connected to eye terminal exposing employees to electrical shock hazards.

29 CFR 1910.305(g) (2) (ii):  Flexible cords were not used only in continuous lengths without splice or tap:    Drill site: On or about August 6, 2014, and at times prior thereto, two flexible cords were spliced together to supply power to the mud logger trailer. This condition exposed the employees to an electrical hazard.
