29 CFR 1910.304(g)(5): Grounding path. The path to ground from circuits, equipment, and enclosures was not permanent, continuous, and effective:  On or about February 27, 2014, Capstar Rig #106, the 70KVA Diesel AC Power Generator #3143 was not grounded properly, the ground electrode rod, approximately 34-inches in length, was inserted approximately 6-inches into the earth. Employees were exposed to shock hazards.

29 CFR 1910.305(g)(2)(iii): Flexible cords and cables were not connected to devices and fittings so that strain relief is provided that will prevent pull from being directly transmitted to joints and/or terminal screws:   On or about February 27, 2014, Capstar Rig #106, the 240V black cable attached to the back of the Mud Logger Trailer #5115 had its strain relief broken which exposed the inner conductors to possible damage. Employees were exposed to shock hazards.
