29 CFR 1910.304(g)(5): The path to ground from circuits, equipment, and enclosures was not permanent, continuous, and effective: On or about July 30, 2014, and at times prior thereto, equipment was not grounded properly exposing employees to electrical shock hazards. 1) Trailer #5001 set up for the mug logger was not grounded properly; the ground rod was only 48-inches in length and inserted 9-inches into the ground. 2) The Whisperwatt generator DCA 70SSIU/Serial #7350794 was not grounded properly; the ground rod was only 48-inches in length and inserted 12-inches into the ground.

29 CFR 1910.305(b)(2)(i): Covers of outlet boxes having holes through which flexible cord pendants passed were not provided with bushings designed for the purpose nor did the holes have smooth, well-rounded surfaces on which the cords could bear: On or about July 30, 2014, and at times prior thereto, the junction box behind Trailer #5001 (set up for the mug logger) was missing bushings on both sides of the box. Employees were exposed to electrical shock hazards.
