29 CFR 1910.305(g)(2)(iii): Flexible cords and cables were not connected to devices and fittings so that strain relief is provided that will prevent pull from being directly transmitted to joints and/or terminal screws: On or about July 31, 2014, and at times prior thereto, the 208V nominal black cable going into the junction box behind Trailer #5025 (set up for the Mud Logger) had a broken strain relief. Employees were exposed to electrical shock hazards.
