29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  a. On or about July 02, 2015, at north side of the residence, two employees were exposed to a fall hazard when ascending and descending from a rooftop on two ladders which were not extended at least 3 feet above the upper landing surface due to their length. The ladder was not secure at its top and a grasping device was not provided.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):     a. On or about July 2, 2015, at the North Side of the two-story house, two employees installing roof shingles were exposed to a 6 feet 6 inches and higher fall hazard to a first level canopy rooftop and 10 feet 2 inches fall hazard from canopy to ground level without the use of personal fall arrest system, safety net system or guardrail system.     Bigfoot Roofing & Construction, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard, which was contained in OSHA inspection number 1031395, citation number 01, item number 003a and was affirmed as a final order on April 13, 2015, with respect to a workplace located at 4544 Alligator Blvd. Middleburg, FL.
