29 CFR 1926.102(a)(1): Eye and face protective equipment were not used when machines or operations presented potential eye or face injury:  a. On a residential roof, at a temporary worksite located at 9417 Arbor Glen Lane in Jacksonville, Florida 32208: On or about June 2, 2023, the employer exposed employees to eye injury hazards caused by airborne debris and flying objects, in that eye protection was not utilized by the employees, while using pneumatic nail guns to install underlayment paper.  Bigfoot Roofing & Construction, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926. 102(a)(1), which was contained in OSHA inspection number 1615942, citation number 01, item number 001 and was affirmed as a final order on November 14, 2022, with respect to a workplace located at 3006 Thorn Glen Road, Jacksonville, FL 32208.

29 CFR  1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels was not protected by guardrail systems, safety net system, a personal fall arrest system, or an alternative fall protection measure as provided by another provision in paragraph (b) of this section:   a. Employees removing and replacing a roof system at elevations of six feet or more above the ground are not being protected from fall hazards by a fall protection system: The employer fails to assure effective protection against falls as required by the standard, as demonstrated on June 2, 2023, when employees removing and replacing a roof system on the 5:12 pitch roof of a one-story residence, located at 9417 Arbor Glen Lane, Jacksonville in Florida 32208, were exposed to the hazards ranging from 9 to 15 foot fall to the ground in that they did not utilize any method or form of fall protection systems.  Bigfoot Roofing & Construction, Inc was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CR 1926.501(b)(13), which was contained in OSHA inspection number 1646426, citation number 01, item number 001 and was affirmed as a final order on March 13, 2023, with respect to a workplace located at 5748 A1A South, Saint Augustine, FL 32080.  Bigfoot Roofing & Construction, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 1615942, citation number 01, item number 002 and was affirmed as a final order on November 14, 2022, with respect to a workplace located at 3006 Thorn Glen Road, Jacksonville, FL 32208.
