At 5748 A1A South, Saint Augustine Florida:  On or about January 25, 2023, the employer exposed an employee to a head injury hazard in that the employee was not wearing a hard hat to protect from falling tools or debris from other employees working on the roof above.

At 5748 A1A South, Saint Augustine Florida:  On or about January 25, 2023, the employer exposed employees to a 25-foot to 30-foot fall hazard in that the roof access ladder side rails were not extended at least 3 feet above the upper landing of the roof.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):   a. At 5748 A1A South, Saint Augustine Florida:  On or about January 25, 2023, the employer exposed employees to a 25-foot to 30-foot fall hazard, in that they were not protected by a fall protection system.  Bigfoot Roofing & Construction, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 1615942, citation number 01, item number 002 and was affirmed as a final order on November 14, 2022, with respect to a workplace located at 3006 Thorn Glen Road, Jacksonville, FL 32208.
