29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):     a. Employees performing roofing work from a 3:12 pitch roof are not being protected from 8 feet, and up to 11 feet 7 inches, fall hazards by a fall protection system.    The employer routinely fails to assure effective protection against falls as required by the standard, as demonstrated on February 17, 2017, when employees were re-roofing a one-story residence at 4180 Heath Rd., Jacksonville, Florida 32277 without fall protections and were exposed to fall hazard of up to 11 feet 7 inches above the surrounding ground.    Bigfoot Roofing & Construction, Inc. previously committed a repeat-serious violation of this standard on July 2, 2015, with respect to a workplace at 96208 Oyster Bay Drive, Fernandina Beach, Florida which became a final order on August 13, 2015 (Inspection No. 1075591).        Bigfoot Roofing & Construction, Inc. previously committed a serious violation of this standard on February 4, 2015, with respect to a workplace at 4544 Alligator Blvd., Middleburg, Florida which became a final order on April 13, 2015 (Inspection No. 1031395).
