An employee was riding in a telehandler's basket, preparing to clean a duct. The forks of the telehandler caught on a beam. The telehandler then backed up. This caused the force on the forks to be released, which jostled the employee. The side of his body bumped into the basket railing and he suffered three broken ribs and a punctured lung. He was hospitalized.

An employee unhitched a tank of anhydrous ammonia. The tank rolled into the employee, crushing the employee's left leg against a pickup truck. The employee's calf was impaled and the employee suffered a fracture in the lower leg.
