On May 19, 2015, at approximately 11:00am, an employee was hospitalized after being struck in the head by a 3 inch rubber air hose fitting. The employee was operating a drill rig at the time of the incident. A hose under 300 pounds pressure, broke at the fitting (swivel came off the threads) allowing the hose to whip and strike the employee in the back of the head.
