An employee was repairing a winch with a ratchet wrench. Tension on the winch caused it and the wrench to spin. The spinning wrench struck the employee's right lower arm and fractured it.

On May 19, 2015, at approximately 11:00am, an employee was hospitalized after being struck in the head by a 3 inch rubber air hose fitting. The employee was operating a drill rig at the time of the incident. A hose under 300 pounds pressure, broke at the fitting (swivel came off the threads) allowing the hose to whip and strike the employee in the back of the head. The employee fell 5' from a platform resulting in a fractured skull.

An employee was inspecting a bridge repair project. A gas-powered, walk-behind pavement saw, being used 12 feet away from the employee lunged forward, cutting his left foot. The employee's fourth toe was amputated and he was hospitalized.
