An employee was using an overhead crane to unload wooden floor boards onto the sub-assembly of a tractor-trailer frame when his left middle fingertip was caught between a load spreader bar and a sub-assembly cross member, requiring medical amputation.
An employee was changing a die in a press when the die slipped and crushed the employee's left index finger. The employee sustained an open facture of the tuft of the left distal phalanx and a partial amputation.
An employee was operating an automated drill and rivet machine and was getting the machine lined up against the crossmember on the trailer side. As the employee was feeling for the crossmember, the machine cycled and the stability bar engaged, resulting in amputation of their index fingertip.
