An employee was changing a die in a press when the die slipped and crushed the employee's left index finger. The employee sustained an open facture of the tuft of the left distal phalanx and a partial amputation.

GREAT DANE LTD. PARTNERSHIP

An employee was operating an automated drill and rivet machine and was getting the machine lined up against the crossmember on the trailer side. As the employee was feeling for the crossmember, the machine cycled and the stability bar engaged, resulting in amputation of their index fingertip. The machine was guarded at the time of the incident.

On May 14, 2021, at approximately 8:30 a.m., an employee was performing sandblasting operations on the inside rear of a trailer. The trailer was secured to a fixture that allows the trailer to be rotated in order to access different parts of the trailer frame. The counterweight had not been added to the frame or fixture and when the employee began to rotate the trailer, the trailer swung down and collided with their left ankle. The employee was hospitalized for an acute displaced fracture of the left tibia and fibula.

An employee was using an overhead crane to unload wooden floor boards onto the sub-assembly of a tractor-trailer frame when his left middle fingertip was caught between a load spreader bar and a sub-assembly cross member, requiring medical amputation.

On May 31, 2016, at approximately 7:00am an employee received a laceration with cuts to the lower artery and three tendons of his left forearm when the 140 lb. bulkhead he was carrying fell from his hands. The employee was hospitalized for surgery.

The employee's right thumb was between the crane's hook and the chain passing through the hook, and when the crane was raised, the distal end of their thumb was crushed. Part of the employee's right thumb was medically amputated.

Two employees were working together to insert a pin into a hydraulic cylinder. One employee was holding the cylinder in place to align it with the pinhole, while the injured employee was inserting the pin. The injured employee was aligning the pin and moved to determine whether the hole alignment was too high or low. At that moment, the crowbar being used to assist with alignment slipped and his left index fingertip was pinched between the cylinder and the housing, resulting in an amputation just below the fingernail.

An employee was hoisting an axle in the buggy department when one side of the sling came loose, causing the axle to tip. The sling hook raised up from the weight transfer, causing the employee's left little finger to be caught by the rigidness of the hook. The fingertip was amputated.

The base plates failed, and one side of the lift dropped. The employee's left leg was caught under the lift basket, and he suffered a fracture to the lower leg including the ankle.
