An employee was operating an automated drill and rivet machine and was getting the machine lined up against the crossmember on the trailer side. As the employee was feeling for the crossmember, the machine cycled and the stability bar engaged, resulting in amputation of their index fingertip. The machine was guarded at the time of the incident.
An employee was changing a die in a press when the die slipped and crushed the employee's left index finger. The employee sustained an open facture of the tuft of the left distal phalanx and a partial amputation.
On May 14, 2021, at approximately 8:30 a.m., an employee was performing sandblasting operations on the inside rear of a trailer. The trailer was secured to a fixture that allows the trailer to be rotated in order to access different parts of the trailer frame.
