Final narrative
On May 14, 2021, at approximately 8:30 a.m., an employee was performing sandblasting operations on the inside rear of a trailer. The trailer was secured to a fixture that allows the trailer to be rotated in order to access different parts of the trailer frame. The counterweight had not been added to the frame or fixture and when the employee began to rotate the trailer, the trailer swung down and collided with their left ankle. The employee was hospitalized for an acute displaced fracture of the left tibia and fibula.

A crew was pulling a 4-inch plastic gas pipe off a reel and straightening it for installation. The injured employee stepped up on the trailer to cut the last band holding the pipe in the coil on the reel. As he turned to step off the trailer, the end of the pipe rotated and sprung out of the cage surrounding the coil, striking the employee on the side of the head and knocking him off of the trailer into the roadway. An employee was fusing 10-inch black rubber utility pipes together using a pipe fusion machine and could not get the pipes to set correctly. The employee used a nylon strap attached to an excavator to lift one side of a pipe off a steel plate. As his hand was between two pipes, the pipes came back together, partially amputating two of his fingers.

Two employees were working together to insert a pin into a hydraulic cylinder. One employee was holding the cylinder in place to align it with the pinhole, while the injured employee was inserting the pin. The injured employee was aligning the pin and moved to determine whether the hole alignment was too high or low. At that moment, the crowbar being used to assist with alignment slipped and his left index fingertip was pinched between the cylinder and the housing, resulting in an amputation just below the fingernail.

An employee was hoisting an axle in the buggy department when one side of the sling came loose, causing the axle to tip. The sling hook raised up from the weight transfer, causing the employee's left little finger to be caught by the rigidness of the hook. The fingertip was amputated.
