Federal OSHA recorded a severe workplace injury at TimkenSteel Corporation, 4511 Faircrest Street SW, CANTON, OHIO 44706 on — Heat (thermal) burns, unspecified, affecting the head and trunk.

During the tap/pour process, an arc flash explosion occurred and an employee that was standing approximately 30 feet from the operation received multiple burns to the face and chest from the molten metal and solids. The employee also sustained lacerations to the thigh and forehead.

An employee was using an oxygen torch to open a tap hole on a ladle. As the employee penetrated the tap hole with the torch, molten steel burned the employee. The employee was hospitalized with second- and third-degree burns to the left hand.

An employee was performing a pre-shift inspection of the straddle carrier truck (PIT). While climbing up the fixed ladder to the cab of the PIT, the employee fell approximately six to seven feet to the floor below. The employee sustained a fractured left heel and a possible compressed disc. The employee was hospitalized and had surgery.

On January 28, 2019, at approximately 1:30 p.m., the injured employee, a scrap operator and employee in training for locomotive operations, was performing scrap railcar spotting operations with a trainer at the half track at Timken Steel Corporation scrap yard. Both employees were riding the leading edge of the railcars proceeding in an easterly direction. The injured employee was riding on the north side of the railcar and the operator was on the south. As the railcar went around a slight bend in the tracks, the injured employee was struck in the lower left leg by a piece of scrap steel. The scrap steel was frozen into the ground and sticking up and was not clear of the railway. The injured employee suffered a broken tibia and fibula that required surgery to repair.

An employee was on a step ladder working on a shaft drive for a cooling bed when he missed a step while coming down, falling backward and hitting his head on equipment or the ground. The injury required stitches.

An employee was setting material in a cutoff saw. The clamp to hold the material in place closed, pinching the employee's left little finger against the material. His fingertip was amputated.

An employee was sitting in the cab of the truck while waste oxidizer was being offloaded. The waste reacted with the receiving tank and an explosion occurred in the tank. Fire traveled through the hose to the tanker truck, causing it to explode. The employee jumped from the burning truck and was hospitalized with burns to his face, chest, and arms.

An employee was installing piping and valves for fire protection systems in the vicinity of an electrical meter outside of a new warehouse. The meter exploded, causing burns to the employee's face and arms.

MI Metals, Inc.

An employee was pouring molten aluminum into a drain pot. The aluminum touched moisture at the bottom of the drain pot and an explosion occurred. The employee sustained burns to their face, hands, and legs.

An employee was operating an oxygen-deficient oven that required manual ignition to make charcoal from wood. As the oven was opened, oxygen was introduced to the environment, resulting in an explosion. The employee sustained head trauma as well as fractures to the arms and legs.

RKM Fireworks Company

An employee was setting up the electrical parts for a fireworks display. They were inserting an e-match when it generated friction with something else. A 3-inch firework shell ignited, injuring the employee's hand and head.

Miba Industrial Bearings U.S. LLC (Grafton)

An employee was maneuvering a 500-pound spin plate from a lower rack. While the plate was being rolled, it went out of control, injured the employee's left bicep, and then rolled over his left foot resulting in a crushing injury to the foot as well as contusions to the torso and left leg. The employee was hospitalized.

An employee was watching a machine run when he became caught in the machine. The employee sustained a severe laceration to the left forearm that partially lacerated a tendon.

An employee was setting up the CNC lathe machine. The employee was adding the tailstock onto the machine when it moved and crushed his right little finger.

NTN-Bower Corporation

An employee was repairing a conveyor hang-up on a machine and pulled up on the belt between the main conveyor drive pulley and the gearbox pulley. When the conveyor hang-up released, the employee's foot slipped, causing their hand to move forward and get pulled into the machine. The employee suffered an amputation to the right ring finger at the first knuckle. The machine was locked out at the time.

NTN-Bower Corporation

On January 9, 2023, at 9:30 p.m., an employee was repairing a robotic machine that had been dropping parts. After making initial repairs, the operator cycled the machine but the problem was still not corrected. The machine operator left the area and upon returning, the injured employee was found pinned by the robot between the grabbing arms and the area where the robot places the parts. The employee suffered a lacerated spleen, lacerated pancreas, a hole in the stomach, a hole in the aorta, and other injuries to the chest and abdomen.

United States Postal Service

An employee was working to deliver mail to an apartment building. She was waiting for a customer to move, to obtain clearance to the mailboxes. The door swung inward and closed on her right little finger. The top half of her finger was surgically amputated.

An employee fell from a step ladder while reaching for a tie-off point. He impacted the ground, and landed on his bolt bag which contained tools. The employee was hospitalized with fractured ribs, and a lacerated spleen and kidney.

GXO Logistics

On October 30, 2025, an employee was working to adjust a stackable metal shipping container. As the container dropped into place, it caught the employee's hands in an area between the upper and lower cross-members. The employee suffered a laceration to the left ring finger that required stitches, bruising to the right ring finger, and fractures to the right middle finger that necessitated medical amputation of the fingertip.

An employee was moving a scissor lift through a doorway. The employee was pinned between the scissor lift and the doorframe, sustained a back injury, and was hospitalized.

American Electric Power Company

An employee was setting up communication equipment for a meeting. They were walking and tripped over a speaker on the ground. The employee sustained a leg injury.
