An employee was on the interior of a building and leaning out the window to weather proof the flange when the employee slipped and fell 14 feet to the ground. The employee punctured a lung and broke the upper left arm, clavicle, upper chest bone, six left-side ribs, and lower back.

On November 10, 2021, an employee was using a chop saw to cut an overhanging stop attached to a door jamb. When the blade hit the jamb, the blade kicked back and his left hand was pulled into it. His left thumb and middle finger were lacerated and his left index finger was amputated below the first knuckle.

An employee was checking an HVAC unit in the attic when they fell approximately 15 feet through the rafters to the floor. The employee sustained fractures to the ribs and scapula as well as a collapsed lung.

An employee was removing grating to change metal chip hoppers located in a pit. The employee lost his balance and fell approximately 12 feet from the grate, landing in the metal chip hopper. The employee sustained a sprained right foot, a left foot fracture, and a fractured L5 vertebra.

The subsequent bending/cutting caused the two overlapping portions to squeeze together and amputate approximately 1/4" of his left index fingertip.

An employee was building wood detail blocking and a wood member needed to be resized. The employee took the wood member to the table saw and set the fence to the correct size. The employee pressed down on the board to control it when his left little finger contacted the running blade, resulting in an amputation.

An employee was clearing a pipe along a culvert using a motorized plumbing machine. The end of the machine got tangled and stuck in long grass and other debris. The employee went to lift the end of the machine out of the grass and clear the blockage when his hand was pulled into the machine, resulting in a partial amputation and open fracture of his left distal phalanx.
