An employee was using a crane. The hook slipped and a fitting fell onto a finger on the employee's left hand causing a fracture and laceration.

An employee was transporting an 800-pound die on a cart from the tool room. The employee was pulling the cart when it struck a metal plate on the floor. This caused the cart to tip forward and the die to slide off the cart. The die struck the employee's left foot and their second toe was amputated.

An employee was using a forklift to load pallets of product on a trailer. He exited the forklift to adjust a pallet by hand. The pallet fell on him, resulting in an injury to the left leg.

An employee was lifting a 500-pound steel counterweight off a pallet using a magnetic hold jib crane. The counterweight detached from the crane and the employee sustained a left foot/toe fracture that required surgery.

An employee was using a pipe wrench to turn a 42-foot-long steel pipe on jack stands to weld the bottom portion. The pipe fell off the stands, striking the injured employee on his left shin. The employee sustained a left leg fracture at the shin area as well as a tibia fracture that required surgery.

A truck driver was preparing a hose to load a chemical tanker trailer with sulfuric acid when sulfuric acid splashed the left side of their face and neck, causing skin burns.

An employee was disconnecting steam lines from a bulk liquid trailer. He disconnected a steam line located on a vapor recovery line when hot steam blew back onto him, resulting in burns to the left side of his abdomen, as well as his left arm and leg. The employee was hospitalized.

An employee was driving a truck when it lost power. He pulled over, opened the hood, checked the engine, and opened the radiator cap. Pressure in the radiator caused hot fluid to spew over the employee, who suffered second-degree burns to the left arm, hand, and leg.

An employee was pulling apart pallets that were stuck together when they felt pain in their stomach. The employee was hospitalized with a hernia.

An employee was getting out of a truck tractor when he fell to the ground. He suffered a broken left femur.

An employee was servicing an exterior mounted environmental control unit while on a 10-foot ladder. He pushed through a pre-made knockout hole on the panel. Upon bringing his arm down, a sharp area along the panel frame lacerated the top of his hand. The employee required surgery.

On 7/31/2025, an employee was training to ride an ATV when it went into a draw and she went over the front of the ATV, landing on the ground. The employee sustained a fractured arm.

An employee was standing behind a frac pump. A discharge hose failed and the employee was burned by hot water on the left upper abdomen. The employee was hospitalized.

An employee working as a contracted firefighter was using a bulldozer to push back an active fire line when they became entrapped in flames, resulting in first- and second-degree burns.

An employee was assisting with the delivery of pipe racks to the drilling rig. The driver was releasing the tie-down lock straps that were holding the pipe racks on the trailer. When the last strap was released, one of the pipe racks rolled off the trailer and struck the employee while they were on the passenger side of the trailer bed. The employee sustained an open wound to the hand and fractures to their left leg and spine.
