An employee was operating a stand-up forklift to move a pallet of product that was sitting on the floor in an aisle of the finishing warehouse next to metal racking. The forklift cab went under the middle shelf and his torso became caught between the metal shelf (at his back) and the forklift steering control stick (at his front). He was hospitalized with possible fractured ribs.

An employee was kneeling on the padded work platform of a casting machine, reaching to pull slack out of a film web being re-threaded through the machine. He lost his balance and slipped, and his right hand, wrist, and forearm were burned in the 90 C hot water bath.

An employee had climbed onto a machine platform and gotten down on his hands and knees to look for the cause of a defect in film that was running on a slitting machine. He slipped and his left hand went into the machine, where it was caught between a roller and a bar. He suffered broken tendons in the hand and wrist.

At approximately 0900, an employee was trying to move a pulp roll back into position on a pulp feed rack. The pulp roll slid off the rack onto the floor and caught the employee's right arm against the rack. The employee was hospitalized with a broken arm that required surgery.

An employee working as a finishing operator was using sand paper to clean a spindle roll on a slitter machine. The employee's arm was caught in the spinning rollers. The employee sustained a compound fracture to the right arm requiring hospitalization and surgery.

An employee was spraying concrete sealer when they fell into a 3-foot drain hole. The employee landed on their feet and sustained a broken femur.

A temporary employee walked across a wooden pallet to retrieve product to stock. When he walked back across the pallet, it broke, causing the employee to fall to the ground and sustain a fractured hip.

On April 10, 2023, at 12:00 PM, an employee was using a broom to clear rock from a building's foundation in preparation for a spray-on vapor barrier. There was a 4-foot deep trench next to the building where the employee was working and the soil at the trench's edge fell away. The employee fell into the trench and was hospitalized for a left pelvis and/or femur fracture.

An employee was walking up a steel staircase to check wiring in the electrical room. The staircase had a top platform with a grate that was sitting across the beams of the platform. The grate had shifted and as the employee stepped on it, it gave way, causing him to fall approximately 3 feet to the ground below. The grate landed on the employee and he was hospitalized for four transverse process fractures in the lower spine.

An employee was stocking the freezer and stepped on a wood pallet. The employee's foot went into the pallet and they sustained a right ankle fracture that required hospitalization and surgery.

An employee opened a screen changer when pressurized steam and resin blew out from the changer, resulting in a fourth-degree burn to their left arm.

A temporary employee cut back-wrapped material on an extrusion line. His left hand was caught in the material as he tried to clear it. As the material continued to rotate, the employee's left ring finger was severed, resulting in a partial amputation.

An employee was using a plastic pipe to clear a jam in the shredding equipment. The pipe got stuck in the shredder and went into a vertical position, crushing the employee's left ring finger between the pipe and the top of the shredder shroud. The employee sustained a fingertip amputation.

An employee was climbing a ladder to get on top of the breakroom. The ladder slipped, causing him to fall to the ground. The employee sustained fractures to his upper back, left foot, and ankle.

Two employees were removing a circuit breaker. The injured employee was using wrenches in the removal process. One of the wrenches contacted an adjacent door, resulting in an arc flash. The employee suffered burns to their neck, face, and ear, as well as momentary vision loss due to the flash.

An employee bent over to pick up a rubber mat from the ground. When he grabbed the corner of the mat, a rattlesnake hidden under it bit his left middle finger near the first knuckle. He was hospitalized.
