An employee was operating a dump pedal during water blasting activities when a 3/16-inch flex lance connection failed, resulting in a puncture wound to the employee's right thigh.

An employee was on horseback roping a calf to be branded. The horse jumped and the employee fell to the ground. The employee was hospitalized for multiple broken ribs.

A crew was preparing a 20-foot section of 12-inch high-density polyethylene (HDPE) pipe to be swung into a welder using an excavator, where it would be welded onto a longer run of pipe. One employee was facing the longer pipe in the welder while the 20-foot section was being rigged. The 20-foot section moved and struck the employee's left knee. The employee's left leg was broken below the knee.

An employee was running pipe into a hole on a rig floor. He was still running the other joint into the hole when the laydown machine shot the joint of pipe. The pipe hit the employee in the jaw and he fell backward, suffering a fractured cheekbone.

An employee was in the yard area on a forklift pulling lumber. Inclement weather began and the employee stopped the forklift and was walking to a safe location when they were struck by flying debris from the high winds or tornado. The employee sustained a leg fracture.

After reassembly work on a ready-mix truck, an employee turned the truck on to check for leaks. The employee was standing on the truck platform, about 3 feet off the ground, when he heard an air leak. As he was turning off the truck, a flying bolt from the plate assembly struck his forehead and broke his skull. He was hospitalized.

An employee was working to remove a product blockage from a piece of equipment. The employee was struck by a metal bar near the deck of the pin mixer equipment and sustained blunt force trauma to the torso.

An employee slipped and fell while exiting the cab of his tender truck. The employee was hospitalized with a fractured femur.

An employee was descending the side of a barge, having ascended to the top of it to close an opening on top of the barge lid. There was a towing rope on top of the three-step ladder on the barge; the employee put his weight on this rope, believing it to be part of the ladder, and fell backward. His ankle and shin were caught between the top of the third step and the barge lid. He suffered a fracture near the right ankle and another near his right shin and was hospitalized, undergoing surgery.

An employee was climbing onto the back of a fertilizer truck and fell to the ground. The employee was hospitalized for broken ribs and a punctured lung.

An employee fell through a hole in a work platform to the ground about 21 feet below. The employee was hospitalized with multiple fractures and a traumatic brain injury.

An employee tripped over a bin and fell to the floor. She landed on her hip and braced herself with her arm, resulting in a fractured hip and wrist.

An employee was diagnosing the lack of flow of product to a powder bin. The employee removed the rotary star valve below the bin. While he was reinstalling the valve, his right middle finger was crushed between its shaft and its housing. The finger was partially amputated.

An employee was standing on a multi-purpose ladder installing head flashing over a window. Their feet were approximately 6 feet off the ground and three rungs from the top. The employee lost their grip on the drill they were using and the ladder tipped over. The employee fell from the ladder to the ground, resulting in a fractured left wrist, bruised chest, and lacerations to their face and lip.

An employee was inspecting the overhead area of a single-person lift and repositioning the lift through a double doorway. As they were transitioning through the doorway, they were caught between the doorframe and the vertical mast of the lift. The employee was hospitalized with injuries to the nose, left maxillary sinus, left orbital bone, and left jaw, with fractures at the maxillary sinus area.

An employee was loading a 4-inch wooden door into a pickup truck when he felt a pop in his right side behind his shoulder blade. He was hospitalized later that day and underwent surgery, having suffered a collapsed right lung.
