An employee was using an overhead crane with a sheet lifter to move two stacks of carbon plates. They were standing in-between the two stacks as the sheet lifter lowered onto one of the stacks. The bundled material slipped out and the employee's right ankle was pinned against the other stack, resulting in a fractured leg and ankle that required hospitalization.

An employee was moving 4-inch pieces of solid round material, cradled in a sling. The employee's right middle finger was pinched between two pieces of the material and partially amputated.

On June 29, 2021, an employee was unloading materials from a flat bed truck using an overhead crane and a nylon sling. While positioning the nylon sling to hoist a stack of 6"x12' aluminum round bars that was on top of 5"x12' rectangular flat bars, the round bars shifted, rolled back, and pinned the employee's left middle finger between a round bar and a flat bar, resulting in a fingertip amputation.

A maintenance employee was making adjustments to the measurements on a tube laser. The employee was inside the inter-lock doors when the machine started up. The employee was struck in the torso by the machine arm and suffered ruptured intestines and internal bleeding.

An employee was making modifications to an electrical panel when an arc flash occurred. The employee suffered burns to multiple parts of the body.

An employee was installing a ground wire to a power transmission pole. The ground wire contacted an energized portion of a cut-out, causing an arc-flash. The employee was hospitalized with second degree burns to their chest and arms.

An employee was connecting a utility transformer for underground service to a home. The employee's impact drill went across two connection bars with 240 volts of potential, creating an arc flash. The employee sustained burns to the face and eyes due to the arc flash and molten aluminum.

On December 15, 2023, at 9:15 AM, an employee was changing 60-amp fuses in a 480-volt panel when an arc flash occurred. The employee was hospitalized with burns to both hands.

An employee had just turned off breakers and was loosening wires on panels when they were shocked by 480 volts of electricity.

A load of steel angles was being lowered when the lifting chain hit the load, causing it to slide toward an employee. An angle slid and pinched the employee, catching both of their legs between the beams and an angle. The employee suffered two fractured legs.

An employee was on the deck of a trailer helping a customer unload tubes. The employee was working to place a cargo strap on the remaining tubes using a winch bar when it slipped off the tubing, causing him to lose his balance and fall headfirst onto the asphalt lot below. The employee suffered a contusion on his forehead, resulting in hospitalization.

An employee and a customer were unloading two rectangular tubes. The tubes were wrapped together. The plastic wrap was cut so that one tube could be picked up. As the employee was lifting the top tube, the other tube (still connected to the top tube) came down onto his hand and broke two of his fingers. He was hospitalized.

An employee was assisting as a forklift got pipe from a pipe rack. The employee was on one of the forklift's forks as it was raised to the top of the rack, a height of about 15 feet. He fell to the ground and suffered a dislocated and fractured right elbow.

On September 23, 2025, at approximately 7:43 p.m., an employee was using a knife to cut poly banding from a metal coil. The knife slipped, causing a laceration to the inside of their left arm at the elbow joint. The employee was hospitalized and required surgery to repair a damaged vein.

An employee was working to deliver mail to an apartment building. She was waiting for a customer to move, to obtain clearance to the mailboxes. The door swung inward and closed on her right little finger. The top half of her finger was surgically amputated.

An employee fell from a step ladder while reaching for a tie-off point. He impacted the ground, and landed on his bolt bag which contained tools. The employee was hospitalized with fractured ribs, and a lacerated spleen and kidney.

On October 30, 2025, an employee was working to adjust a stackable metal shipping container. As the container dropped into place, it caught the employee's hands in an area between the upper and lower cross-members. The employee suffered a laceration to the left ring finger that required stitches, bruising to the right ring finger, and fractures to the right middle finger that necessitated medical amputation of the fingertip.

An employee was moving a scissor lift through a doorway. The employee was pinned between the scissor lift and the doorframe, sustained a back injury, and was hospitalized.

An employee was setting up communication equipment for a meeting. They were walking and tripped over a speaker on the ground. The employee sustained a leg injury.
