At 6:24 p.m. on October 7, 2021, an employee was unplugging a personnel carrier that was parked and being charged. The cart unexpectedly moved forward and pinned the employee's left leg against a support column. The lower leg was broken and the employee was hospitalized.

On September 13, 2025, an employee had been working at an outdoor event. At the end the day, she was walking around outside the facility to make sure everything was put away and felt overheated. The employee was hospitalized for heat-related illness.

On September 5, 2025, at approximately 7:35 AM, a temporary employee was at the engine block rollover machine. Their left index fingertip was caught and amputated between a pneumatic part and a guard.

On June 27, 2025, at approximately 8:35 AM, an employee was operating a box maker machine when their left middle fingertip was caught by a chain and sprocket. The employee's fingertip was partially amputated without loss of bone or the fingernail.

On December 13, 2023, an employee was operating a forklift. When they stepped off the forklift, it crushed their left foot between the lift and a racking system. The employee suffered a foot fracture that required hospitalization.

An employee was maneuvering a motorized pallet jack while standing in front of it. He became pinned between the pallet jack and a steel rack cross-beam, resulting in a contusion to his chest/back and a fractured rib.

The Whitaker Company

An employee was stacking empty drums into a trailer. The employee then exited the trailer, using the forks of a forklift as a step. The forklift's parking brake failed, causing the forklift to move forward and pin the employee's right leg between the forklift and the trailer. The employee's right lower leg was crushed, resulting in a fracture and a laceration.

An employee had been spraying weeds behind a school. As he was leaving the field, the employee drove his truck up to the gate, got out, and opened the gate. The truck rolled forward and pinned him against the fence/gate. The gate then gave way and opened from the force of the truck. The employee's left index fingertip got caught in the fence/gate and was amputated.

PactivEvergreen, Inc.

An employee was using a walk-behind tugger to pull a grinder. The employee's left little finger was pinched between the handle of the tugger and the framework of a staircase. The employee's fingertip was amputated.

Benton Foundry, Inc.

An employee was realigning two halves of a core box on a core-making machine. Pneumatic pressure released, causing the core box to close on the employee's right thumb. The thumb tip was amputated between the nail and the knuckle.

American Cast Iron Pipe Company

An employee was performing maintenance/electrical work inside a substation, and used a ladder to access the top of a panel box. The employee fell 8-9 feet from the top of the panel box to the ground. The employee was hospitalized with fractures to their ribs and a shoulder injury.

NIBCO INC.

An employee's hand was caught in a pinch point in the area of a conveyor for a flask running out of a foundry molding machine. The employee's ring finger was crushed and partially amputated at the base.

American Castings, LLC

An employee noticed a block was out of place under the weighted mold frame. The employee used another block to push the block out and replace it. When the block came out, the frame came down on his right ring and little fingers. The employee sustained a fracture and partial amputation to the ring fingertip.

OSCO Industries Inc.

An employee was using a fork truck to move a tub containing molten iron that had been drained from the cupula. The tub tipped over and the molten metal came in contact with water. This caused the molten iron to splash back and burn the employee's upper torso.

An employee fell while moving a water canister, resulting in fractures to four fingers on her right hand.

An employee was changing a die in a press when the die slipped and crushed the employee's left index finger. The employee sustained an open facture of the tuft of the left distal phalanx and a partial amputation.

An employee was mixing sauce using a sauce barrel mixer when his left little finger was amputated.

AbbVie Inc.

An employee was driving a boom lift (in the lowered position) in an exterior dock area. The lift s left wheels rolled onto base plates that covered a 3-foot-deep sump pit. The base plates failed, and one side of the lift dropped. The employee's left leg was caught under the lift basket, and he suffered a fracture to the lower leg including the ankle.
