An employee was standing on a loading dock near the end of a trailer. As the employee was stepping backward, they were struck on their right side by a crate that a forklift was transporting. The employee was pushed off balance and into a steel beam on their left side. They suffered a broken left arm.

An employee was delivering material to a jobsite, slipped off the lift gate of a truck and fell to the ground. The employee suffered a hip injury.

An employee was on top of a roof changing a light bulb. The employee fell through the roof to the concrete floor about 25 feet below, suffering a broken neck and three broken back vertebrae.

An employee opened a trailer's rear door and was struck on the right foot by forklift blades that had been stored on a pallet and shifted during transportation, resulting in a contusion.

At 5:08 a.m. on March 16, 2022, an employee was operating a forklift near a dock. The forklift backed out of the dock door into an empty space without a trailer and fell to the ground about 4 feet below. It then rolled onto its side and caused fractures to the employee's right hand, left leg, and left foot. The employee was hospitalized.

An employee was cleaning off a pallet sorting machine. The employee jumped approximately two feet from the top of the pallet sorting machine to a lower platform. Upon landing, the employee's left ankle rolled, resulting in a fracture that required hospitalization.

An employee was working from the forth rung of an 8-foot step ladder installing radiator covers when the cover began to fall. The employee lost balance and jumped from the ladder, landing on his feet and sustaining a heel fracture.

An employee was standing on their flatbed truck adjusting a cargo pallet. He pulled one of the pallet boards and the board broke, causing the employee to lose his balance and jump 4 feet off the flatbed. The employee landed on a concrete surface and sustained a fractured left heel that required surgery.

An employee was cleaning a single screen machine that had been jammed by a garden hose. The employee exited the machine onto the belt guard and jumped from the belt guard to the ground three feet below. When he landed, he broke his left femur.

An employee was on a catwalk helping to lay down tubing when he lost his balance. The employee jumped approximately 42 inches to the ground and the 30-foot section of tubing struck his left shin resulting in a fracture that required surgery.

An employee was changing a die in a press when the die slipped and crushed the employee's left index finger. The employee sustained an open facture of the tuft of the left distal phalanx and a partial amputation.

An employee was assisting with rigging a magnet to an overhead crane in order to lift a sheet of steel. The employee's right thumb was between the crane's hook and the chain passing through the hook, and when the crane was raised, the distal end of their thumb was crushed. Part of the employee's right thumb was medically amputated.

Two employees were working together to insert a pin into a hydraulic cylinder. One employee was holding the cylinder in place to align it with the pinhole, while the injured employee was inserting the pin. The injured employee was aligning the pin and moved to determine whether the hole alignment was too high or low. At that moment, the crowbar being used to assist with alignment slipped and his left index fingertip was pinched between the cylinder and the housing, resulting in an amputation just below the fingernail.

An employee was hoisting an axle in the buggy department when one side of the sling came loose, causing the axle to tip. The sling hook raised up from the weight transfer, causing the employee's left little finger to be caught by the rigidness of the hook. The fingertip was amputated.

An employee was using a circular saw to cut the cap off an aluminum bulkhead of a trailer that was being manufactured. The saw kicked, lacerating his hand and resulting in the amputation of his left middle and ring fingertips.

An employee was operating an ironworker (steel punching/shearing) machine. A piece of the tooling broke off and struck the employee's neck; a fragment was lodged in his neck. The employee was hospitalized and required surgery.

On October 25, 2025, an employee was stringing up a new reel of tire tread on a machine. The employee pulled a 6-inch tail and proceeded to spool up the liner. As the machine was jogged to begin the liner wrapping process, the liner caught the employee's glove and pulled their right arm into the liner, causing a fracture to the arm.

An employee was doing a cable change on top of an electric overhead traveling (EOT) crane. They were positioned between the cable drum and the drive shaft. While rotating the cable drum, the employee reached for the new cable that was to be installed. The grease fitting on the drive shaft caught their fall harness and pulled them underneath the drive shaft. The employee was pinned between the drive shaft and two pieces of angle steel, resulting in a fracture to their left hip.

An employee was helping two other employees to push a pallet of tile on a loading dock. He suffered a broken fibula and was hospitalized.

On October 5, 2025, an employee was pulling a pressure washer hose when it it disconnected from the cooker. The employee was struck by jets of high-pressure water, resulting in lacerations to their left thigh requiring surgery. The employee was hospitalized.
