Final narrative
An employee had stepped into a company van to retrieve hardware/parts. As he was stepping backward out of the rear of the van, his pant leg snagged on the trailer hitch under the bumper. The employee fell backward out of the van, landing on his left hand and arm and sustaining fractures to the left distal radius and ulnar styloid.
Two employees were preparing two industrial control panels for shipment. The control panel is 5 feet wide, 7 feet tall, 1 foot deep, and weighs approximately 500lbs. They were setting one on the shipping pallet when the second one fell on top of the worker striking him on the back and injuring his right shoulder, ribs, and lung.
An employee was on an A-frame ladder retrieving materials from a shelf. He slipped and fell and his leg got caught in the ladder rungs, breaking his left tibia.
