QG Printing II, LLC

At about 3:30 p.m. on March 20, 2021, employees were changing the lap on a signature on a printing press. An employee was checking to see that the lap was flush when his right index finger was caught under a plate. The fingertip was amputated.

On February 23, 2019, an employee was removing dust from the sensor of a recycling system. The fingertips on the employee's right hand came into contact with the paddle blade, causing amputations to the middle and ring fingers.

An employee stepped onto the plexiglass window on the binding machine. The window broke and the employee fell three feet suffering a broken leg.

An employee was preparing to clean the ink tray of a printing machine when he stumbled and put his left hand out to catch himself. His hand entered the machine's unguarded rollers and was de-gloved.

An employee had just replaced the passenger-side rear panel of a stake truck and was latching the panels together. A freight truck pulled away from the loading dock and struck the vehicle the employee was working on, pushing the vehicle into the employee and pinning him between the vehicle and a roll-off dumpster. He suffered a hip fracture.

Employees were setting up to offload an aircraft. The injured employee was standing near a baggage tractor by the right wing of the plane. A belt-loader was being driven from the nose of the aircraft to the aft when it struck the parked baggage tractor. The injured employee was pinned between the belt-loader and the baggage tractor and sustained a dislocated left knee.

An employee was hand-picking orders while standing near an order picker forklift. A co-worker was operating a turret forklift nearby when it struck the order picker forklift, which rolled over the employee's left foot. The employee suffered an injury to their big toe that required hospitalization and surgery.

An employee was walking between the tines of a stationary forklift when a nearby forklift struck the stationary vehicle, causing the tines to strike the employee. The employee was hospitalized for a leg injury.

A car wash employee was preparing a vehicle for a wash when a second vehicle pulled behind it and accelerated, striking the vehicle in front. The employee's left leg was struck, resulting in left lower leg and ankle fractures.

An employee was removing packaging from a roll of printing substrate. The blade of their utility knife got stuck. The employee used both hands to free the knife and the blade partially amputated their left little fingertip.

The injured employee's standup forklift had mechanical issue with a tire. The injured employee was inside their forklift to help steer it while it was being pushed up the ramp by another forklift. As the other forklift started to move up the ramp, it's forks moved and went into the driver's space of the disabled forklift. The fork penetrated the injured employee's left shoe, fracturing the two middle toes and injuring the first toe. The employee was hospitalized and required surgery.

A press was having a paster failure and the crew was working to get the ink on the paper. A press operator grounded himself and was entering the press unit when something flashed. The employee sustained third-degree burns to his head and left arm.

Around 12:45 PM, a clamp truck operator went to remove a piece of orange strap from a clamp truck that was parked outside of the office. The strap fell into a gap between the frame and the hood of the truck. The strap became caught in the engine's fan and also wrapped around the employee's hand when he went to grab it. The strap was pulled tight against the frame of the clamp truck, causing lacerations and fractures to the employee's hand. The employee was hospitalized and required surgery.

At 12:00 p.m. on August 13, 2025, an employee was unloading a 1,100-pound roll of paper from a skid when the roll fell onto his right leg. The leg was broken.

An employee was helping a coworker transport a tall palm tree with a mini skid steer. The employee was severely shocked by a high-voltage electrical wire above the ground.

An employee was retrieving a Christmas tree from a shelf using a ladder. He missed a step and fell to the concrete floor. He sustained injury to his head and wrist.

An employee was inspecting a generator whose radiator was leaking. He slipped, and a fan blade in the generator amputated his thumb and index finger.

An employee was operating an agricultural tractor during sugarcane harvesting. The employee sustained a lumbar sprain due to vibration or motion from the tractor.

An employee was moving a 3-ton condensing unit, strapped down on a dolly, out of a garage. The strap broke, causing the employee to fall backward onto the brick pavered driveway. The employee suffered injury to a spinal ligament in the neck.
