An employee was unloading a ship when he was struck by a forklift, resulting in a broken right ankle.

An employee was scanning barcodes on vehicles and going through the manifest as a checker. They went to the rear of a vehicle to scan a barcode when the vehicle behind them rolled forward into them. The employee was punctured by a removeable latching pin on the front of the vehicle and sustained a laceration above the knee. They were hospitalized.

On September 8, 2022, an employee was descending the ladder of a cargo ship when he fell approximately 4 feet to the deck below. The employee was hospitalized with a neck fracture, head laceration, and bruises on his right leg.

An employee was standing next to a jockey truck. A pickup truck crossed a marked RTG lane and struck the employee, who suffered a fractured skull, broken vertebrae in the back, and a dislocated left shoulder.

A truck driver had parked a truck, carrying a container, under a crane. The crane picked up the trailer and took the truck with it, lifting the truck about 10 feet. When the crane lowered the truck, equipment inside the cabin injured the employee's neck, lower back, and right arm. The employee was hospitalized.

An employee was walking into the motor control center (MCC) room when his right ring finger was caught in the hinge of a doorway. He sustained an open phalanx fracture, which resulted in a partial amputation above the first knuckle.

An employee was stacking concrete blocks for a concrete bunker. As a block was swinging into place, the employee's left leg was pinched between two blocks, resulting in injuries to his left heel and lower leg.

An employee was using a crane to move a 44-foot, 3,343-pound I-beam. As the beam was moving west, it straightened out (north to south), beginning to swing south toward the northwest corner of a building. The crane then sent the beam southward, directly toward the corner of the building. The beam caught the tips of the employee's left index, middle, and ring fingers against the building. The last joints of the middle and ring fingers were amputated, and the last joint of the index finger was partially crushed.

An employee was placing a board on top of a bundle of steel tubing. A crane was moving a bundle of steel that struck the employee and caught them against another bundle of steel tubing. The employee sustained a crushing injury and hematoma to the right upper leg.

Employees were using a crane to lower large tree stems and branches while removing limbs and branches from the load. This caused the load to shift and strike an employee in the back, resulting in a hematoma on the lower left part of their back and a fractured ankle.

While preparing to install a new spool piece for a vapor and spent sulfuric acid line, an employee was working to ensure there was no residual product or pressure in the line before removing the 6-inch hose. The valve was cracked, and some spent acid still in the line sprayed out. The employee was standing off to the side and at a distance, but was still contacted by the hot acid, resulting in corrosive and thermal burns to their upper torso and mid-thighs of both legs.

An employee was driving a utility tractor rig when it struck the rear trailer of another utility tractor rig, which was traveling in the opposite direction. The glancing impact caused damage to the cab, and he sustained a laceration on right side of his face, a cervical fracture, two fractured left ribs, and a pelvic fracture. The employee was hospitalized.

The injured employee was standing by the controls for the electric spud system on the barge, preparing to raise the spuds. Meanwhile, an excavator was moving a mat on the deck. As the excavator swung from port to starboard, it's counterweight struck the injured employee s shoulder, causing them to lose balance and fall against the spud controls. The employee sustained bruises and contusions to their right shoulder and chest.

An employee was standing on the outside edge of a hopper barge while watching a skid steer push ore for an excavator (with a clamshell bucket) to unload. The excavator was on a work barge. The bucket of the excavator struck the employee and caused a near-amputation of their right arm and fractures of the ulna and radius of the left arm. The right arm required surgery.

An employee was inspecting a conveyor to see why it was not running. The employee entered the the conveyor motor area, tripped on a piece of angle iron welded to the floor, and fell. The employee sustained a fractured orbital bone.

An employee delivered a package. On her way back to her vehicle she was attacked by two dogs that came from around a corner. She was hospitalized with dog bites.

The injured employee was standing by the controls for the electric spud system on the barge, preparing to raise the spuds. Meanwhile, an excavator was moving a mat on the deck. As the excavator swung from port to starboard, it's counterweight struck the injured employee s shoulder, causing them to lose balance and fall against the spud controls. The employee sustained bruises and contusions to their right shoulder and chest.

A technician was operating a crane hoist to lift/remove a cylinder head from the main engine (port). The load was lifted at an angle, causing the cylinder head to get stuck on the cylinder head studs. When it became free from the studs, the cylinder head swung. It struck the technician and caught them against the cabinet door. This resulted in an amputation of their left index fingertip at the distal knuckle.

An employee was securing a power cord to a refrigerated truck container. A truck started backing up to connect with the container and the employee was pinned between the tires of the container and the bolster of the truck, resulting in a fractured leg.

An employee was delivering a package to a house when a dog came through the storm door, jumped on the employee, and bit them in the lower jaw.
