The empty beam, which is stored overhead, came down as the employee's left hand was guiding the completed beam. This resulted in the amputation of the little finger.

The rubberized coating on the exit roller ruptured, blowing pressurized steam and hot water onto the employee. One employee was hospitalized with chemical and thermal burns to their upper back and eyes.
