An employee was winching in the mid-section of a boom on an aerial lift into the base section when their left middle and ring fingers were pinched between the two sections of the boom subassembly. The employee sustained a displaced fracture of the middle phalanx of their left middle finger and the amputation of their left ring fingertip.

An employee was preparing to use a programmable automated vertical band saw. The employee was clamping a piece of angle iron in place on the machine when the clamp caught the employee's left index fingertip. The employee suffered an amputation to the fingertip, up to the edge of the nail.

An employee was preparing a new lift to receive an engine. His right pinky finger was pinched and he sustained a loss of soft tissue.

His little finger was pinched between the lifting hook and the steel part, causing the amputation of the tip.

The employee suffered injuries including broken ribs, spinal fractures, scapula fractures, and internal injuries.

A technician was operating a crane hoist to lift/remove a cylinder head from the main engine (port). The load was lifted at an angle, causing the cylinder head to get stuck on the cylinder head studs. When it became free from the studs, the cylinder head swung. It struck the technician and caught them against the cabinet door. This resulted in an amputation of their left index fingertip at the distal knuckle.

An employee was closing a door when the door closed on their right index finger, resulting in a fingertip amputation.

An employee was walking into the motor control center (MCC) room when his right ring finger was caught in the hinge of a doorway. He sustained an open phalanx fracture, which resulted in a partial amputation above the first knuckle.
