Final narrative
An employee was using an overhead crane to move steel tubing when one stack of tubing fell and struck the employee. The employee was hospitalized with a fractured left leg.
An employee hooked up two bundles of steel tubing to be lifted with an overhead crane. He stumbled while walking and operating the crane and his right hand contacted the crane's pulley mechanism. The upward motion of the crane caused the cable to pull his finger into the pulley. The employee's fingertip was partially amputated.
An employee was pulling a piece of square bar (weighing about 354 pounds). The bar slid and pinched the employee's right middle fingertip against another bar, causing an amputation (as well as a laceration and fracture) to the fingertip.
An employee was pulling a nylon sling up from between loose pieces of material in a bundle cart. The bundle cart tipped over, and seven loose pieces of tubing fell from it to the ground about 30 inches below, then rolled into the employee's left ankle/foot. The employee was pinched between the tubing and a stack of material.

On June 27, 2025, at approximately 7:00 PM, an employee was operating a cell that presses rings on a cylinder and welds the rings on. The cylinder was moving between the welding station and the press station when it fell off the holder. The employee went to place the cylinder back on the holder when the cell cycled and the compression holder crushed their right hand. The employee's middle, ring, and little fingers were crushed.
An employee was working on a hydraulic cylinder at his work table. While he was removing a bleeder plug to reduce the pressure, the 3/8-inch steel plug was pushed out by oil pressure. The plug impacted his wrist, causing an abrasion where hydraulic oil went into his arm. The employee was hospitalized.

An employee was changing a die in a press when the die slipped and crushed the employee's left index finger. The employee sustained an open facture of the tuft of the left distal phalanx and a partial amputation.
An employee was driving a boom lift (in the lowered position) in an exterior dock area. The lift s left wheels rolled onto base plates that covered a 3-foot-deep sump pit. The base plates failed, and one side of the lift dropped. The employee's left leg was caught under the lift basket, and he suffered a fracture to the lower leg including the ankle.
