Employees were in a mobile crane when it collapsed. They fell approximately100 feet to the ground and two employees were hospitalized. One employee sustained a hip injury and the other employee sustained a lung injury.

Employee 1 had just finished disassembling a metal girder structure on the ground and had removed the bolts from its metal cross brace. Employee 1 then moved to the left side of the structure. Employee 2 was using a skid steer to pick up the metal cross brace. Once the cross brace was on the forks of the skid steer, employee 1's right leg was caught between the cross brace and the metal girder structure. Employee 1 was hospitalized with a fractured right leg, which required surgery to repair.

Employees were replacing an overhead utility line switch with a new set of switchgear. The injured employee was tightening a bolt to complete the last step of the work when an arc flash occurred and the employee was shocked.

Employees were performing water-based excavation using a hydrovac truck. The stream of pressurized water lacerated an employee's left foot.

Employees were removing electrical cables for replacement. When one employee cut a cable, the employee was shocked in the abdominal area.

The employee was rigging a towing strap to a front-end loader to tow a truck out of the mud and onto matting. The front-end loader moved while the employee's thumb was between the vertical drop pin and the tow bar of the front-end loader, resulting in a partial thumb tip amputation.

An employee was installing metal panels on a roof when they fell through a skylight and landed on the ground 40 feet below. The employee sustained injuries to their head and ribs.

Otis Elevator Company

Employees were installing components of an elevator from inside the elevator and removed temporary hoisting equipment when the elevator descended from the fourth floor to the pit floor. One employee was hospitalized with fractures to their fibula and a rib, and the other employee was hospitalized with fractures to both feet.

An employee was descending after trimming up in a tree when the limb that his climb line was attached to failed after he descended 7 feet. The employee fell approximately 30 feet to the ground. The employee sustained three fractured vertebrae and a fractured sternum.

An employee was working 32 feet above the ground in a scissor lift, attaching a camera to a gantry crane. The crane moved and struck the scissor lift, which tipped and fell to the ground. The employee was hospitalized with fractures to his collarbone, humerus, and pelvis. Fall protection was worn at the time.

An employee was descending the side of a scaffold when a rail detached from the scaffold and the employee fell 38 feet to the concrete ground. The employee sustained a fractured pelvis, fractured lumbar vertebra(e), and lacerations.

HB Fleming, Inc.

A crew was installing drilled micropiles alongside an outdoor covered deck foundation. The injured employee was working the front of the drill when a loose section of casing dropped onto the tip of a rig wrench and pulled the wrench down to an embedded casing. The employee's left little finger was caught between the rig wrench and the embedded casing and was amputated above the top knuckle.

An employee was standing on an extension ladder, using a torquing tool to remove bolts that secured blades to a rotor. When the torquing tool activated, its reaction arm came around and pinched the employee's right middle finger against a lifting eye. His fingertip was amputated.

Ahtna Marine and Construction Company, LLC

Employees were securing the fuel line of an outboard motor in preparation for removing the motor from a small vessel located in the equipment yard. A gasoline-related fire occurred and one employee sustained burns to both hands and forearms.

Michels Construction, Inc

A marine diver/construction worker was working underwater, using a hydraulic dredge to clear sediment away from the base of a dam. The dredge's suction nozzle began migrating toward him, and then pulled in his right hand and forearm. He sustained compartment syndrome in the hand and forearm.

Bechtel Energy Construction Services, Inc.

An employee was standing on a sheet of -inch by 4-foot by 8-foot plywood on top of a rebar mat, supervising his crew. As he was moving to alert the crew to an incoming overhead load, he stepped off the plywood and his left foot slipped through the rebar mat and landed on the rebar mat below. His left ankle was broken and dislocated.

An employee was helping a coworker transport a tall palm tree with a mini skid steer. The employee was severely shocked by a high-voltage electrical wire above the ground.

An employee was retrieving a Christmas tree from a shelf using a ladder. He missed a step and fell to the concrete floor. He sustained injury to his head and wrist.
